To fill in the Pre-Admission paperwork for surgery online, go to:
- Click on the blue top tab ‘For Patient” and then click “Pre-Admission”.
- Scroll down the page and click on the orange “Admission Form” button, and follow the prompts to begin.
North Shore Private Hospital

- Click on the orange “Admission Form” button and then click “All Other Patients”.
- Click on “Start New Form” and follow the prompts to begin.

For admissions to public hospitals, please discuss the admission pathway with your doctor, or one of our nurses. It may require a pre-operative clinic visit to that specific public hospital in order to fulfil the necessary paperwork
